Job Description About the Role Are you an experienced Registered Nurse looking for clinical autonomy, program leadership, and the flexibility to work from home? Well Life Medicine is an independently owned, multidisciplinary Primary Care Home in Oregon. We are seeking a dedicated RN to take full ownership of our Chronic Care Management (CCM) program. In this hybrid role, you will be the primary driver of our chronic care initiatives—managing the patient panel, executing billing, and performing face-to-face care coordination. Program Ownership: You will have the autonomy to build, manage, and optimize our CCM program without being micromanaged. Key Responsibilities Own the CCM Program (Remote/Hybrid): Maintain and manage the active list of CCM patients. Ensure all monthly patient needs are addressed and independently manage the monthly CCM billing cycles. In-Person Care Coordination (1-2 days/week in clinic): Conduct face-to-face visits for complex and simple chronic care coordination. Personally meet with patients to establish them into the WLM CCM program. Provider Collaboration: Work directly with our primary care providers to evaluate, adjust, and optimize patient care plans. Triage & Transitions: Assist providers with patient phone triage, medical advice, and medication refill requests. Act as the vital link for patients discharging from the ED or hospital to ensure safe transitions of care. Documentation: Maintain highly accurate clinical and billing documentation within our Athena EHR system. Qualifications Valid Registered Nurse (RN) license in the state of Oregon. 1 year of relevant clinical experience (Previous experience with Chronic Care Management workflows and billing is highly desirable). Strong independent workflow skills—self-motivation and highly organized communication are essential for the remote days. Familiarity with Athena EHR and outpatient care coordination is a major plus.
