Provide initial and on-going assessment and evaluation of patient response to therapy. Accurately document evaluation and treatments provided. Attend trainings to build understanding and capacity to undertake QAPI work. Carry out QAPI roles and responsibilities as assigned. Our Home Health Occupational Therapist is responsible for the following: Providing services in accordance with accepted standards of practice for occupational therapy. Documenting all therapy interventions. Providing initial and ongoing assessment and evaluation of patient/family response to therapy. Developing and revising the plan of care in consultation with the physician and other care team members. Providing therapeutic treatments and evaluation of equipment needs as applicable. Advising and consulting with the family and other agency personnel as applicable Completing documentation timely for all assessment and evaluation data, treatments, and patient's response to therapy intervention. Evaluating outcomes of care and completing the OASIS assessment accurately. Providing discharge planning and evaluation. Supervising occupational therapy assistants at least every 30 days and home health aides every 14 days when appropriate. Participating in in-service programs. Participating in IDT and caseload discussions. What You Have: Must have graduated after successful completion of an occupational therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organizations of ACOTE. Must be eligible to take or have successfully completed the entry-level certification examination for occupational therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT). Must have a current and valid PA Driver's License. Must hold proof of motor vehicle insurance. Must have Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers.
