Time) opportunity in West Chester, PA Great part-time (2 days per
week) primary care role managing complex patient cases in a
post-acute setting (SNF) in West Chester, Pennsylvania, focusing on
improving care for some of the most vulnerable patients. Position

Qualifications: Minimum of 1 year of experience, with a commitment
to delivering high-quality, patient-centered care Post-acute care
experience is beneficial, though not required, as we provide

unrestricted DEA/prescriptive authority (or in process) A deep
commitment to improving patient outcomes, leading clinical
