Western Baltimore and Howard County, MD.Home Health PT Job Details:Pay Rate: $2240-$2440 gross weeklySchedule: 8a-5pAvailable Shift: 8 hour shifts, 5 shifts per weekStart Date: 5/12/25Located In: Baltimore, MDRequirements: Qualified candidates MUST have at least 2 years experience in a Medicare certified Home Health facility, and a valid PT license and be willing to obtain MD licensure.As a Home Health Physical Therapist (PT), you'll have the opportunity to provide personalized physical therapy services to patients in the comfort of their homes, fostering independence and improving their quality of life. This one-on-one healthcare approach allows you to connect and collaborate with individuals on their care, while acting as an advocate for their needs and quality of life. Responsibilities include:Conducting thorough assessments of patients physical abilities and developing individualized treatment plans to address their unique needsAdministering skilled physical therapy interventions, including therapeutic exercises, manual techniques, and gait training, to promote mobility and functional independenceEducating patients and their families on proper techniques for performing exercises and activities of daily living, as well as strategies for fall prevention and safetyDocumenting patient evaluations, progress notes, and treatment outcomes accurately and promptly in electronic medical recordsMonitoring patients progress and adjust treatment plans as needed to achieve therapeutic goals and optimize outcomesIn the Home Health setting, you will be a vital link between a patient and their family to resources across the care spectrum.
