Job Description:Under general supervision, provides professional
occupational therapy services to patients in their home, to assist
them in achieving maximum potential in functional activities in
accordance with their diagnosis, the physician's orders and
corporate policy and procedures. Responsible for accepting patient
referrals, evaluating for services, and managing a caseload.
Provides consultant occupational therapy services, when
appropriate.
Duties and Responsibilities
- Related to Patient Care
- Provides patient and family-oriented care through a process of
occupational therapy evaluation, assessment, planning, and
implementing rehabilitative programs based on identified patient
and family needs according to established short- and long-term
goals.
- Assist the patient's physician and/or VNA staff in evaluating
patient's level of function by applying muscle, joint, sensory,
perceptual and functional ability tests.
- Conducts the initial evaluation visit, within a time frame
appropriate to the patient's needs. The evaluation report will
include the specific procedures and modalities to be used and the
amount, frequency, and duration.
- Develops a written occupational therapy plan of care in
accordance with physician's orders and patient and family needs.
- Carries out occupational therapy programs and procedures
according to established plan of care, incorporating input from
other disciplines involved in the care of the patient.
- Periodically reassesses patient's response to treatment,
changes in psycho-social and/or physical condition, reporting same
to primary nurse, Clinical Management, or physician.
- Educates, instructs, and/or demonstrates techniques of
rehabilitation, equipment use, and home modifications to the
patient/family, providing written material for subsequent
reference.
- Participates in interdisciplinary patient care conferences for
coordinated planning, goal setting, and problem solving.
- Utilizes appropriate disciplines and resources for optimal
patient care.
- Prepares clinical notes, progress notes and discharge
summaries, in accordance with policies.
- Discontinues occupational therapy services, when appropriate,
notifying primary clinician and/or Clinical Management and
physician. Instruct the patient/family how to access follow-up.
- Related to Staff
- Functions as a member of the interdisciplinary team,
encouraging and maintaining open communications at all times.
- Provides clinical supervision to the Home Health Aides and
Occupational Therapy Assistant for assigned patients and provides
input regarding their performance to the appropriate Manager.
Countersigns all clinical notes of Occupational Therapy Assistant.
- Assists in the orientation of new staff or continuing education
of existing staff, as requested.
- Related to Corporate Management
- Submits patient visit schedule to the scheduler on a weekly
basis and calls to inform of schedule changes and to receive new
referrals.
- Participates in the utilization review/quality assurance
program.
- Functions in a cost-effective manner regarding utilization of
time, mileage and supplies.
- Meets or exceeds established productivity and quality assurance
standards.
- Seeks consultation and direction from management personnel as
appropriate.
- Informs Clinical Management of perceived problem areas related
to patient care, corporate management or community relations.
- Keeps clinical skills current with occupational therapy
practice norms.
- Related to Community
- Demonstrates a professional attitude, sound judgment and
continuity in interactions with physicians, other health care
providers, and community agencies.
- Represents the VNA at community activities, as delegated.
- Promotes a positive corporate image.
- Acquires and utilizes knowledge of community
resources.Qualifications
- Minimum Education:
- Bachelor's Degree Graduate of an accredited program in
occupational therapy
- Required Certification:
- Licensed Occupational Therapist
- BLS certification
- Minimum Experience Required:
- 12 months+ clinical experience and rehabilitation
experience
- Skills:
- Possesses a sound knowledge of physical therapy concepts
applicable to the home setting.
- Understanding of and commitment to a holistic approach to
patient care with an understanding of family dynamics.
- Demonstrated clinical, interpersonal, and communication
skills.
- Ability to demonstrate respect and appreciation for other
professionals and paraprofessionals and to work cooperatively.
- Ability to assess, analyze and plan according to need,
utilizing the problem-solving approach.
- Willingness to request and accept consultation and
direction.
- Must possess a valid driver's license in the current state of
residence. Driving record must meet requirements established by
WellSpan Risk Management. Willing/able to drive in varying weather
conditions and in urban/rural settings.
