Job Description:
Job Summary: Evaluates the clients referred to the agency;
assists with insurance coverage, plans, and coordinates care for
non-medical services. Responsible for managing all aspects of
agency marketing, establishing and maintaining positive
relationships with customers and referral sources, and responding
to customer requests and concerns.Qualifications: - Has two year
associate degree at a minimum, four year degree preferred in social
work, nursing, or health-related field. Two years experience in a
health care setting preferred. -Is self directed with the ability
to work with little supervision. Has excellent observation skills.
Is flexible and cooperative in fulfilling role obligation. Has good
oral and communication skills. Has good organizational
skills.General Duties:Evaluates clients referred for non-medical,
non-skilled private duty services.Assists with identifying and
verifying insurance coverage for non-medical or respite
care.Obtains and documents prior authorization for private duty
services from insurance providers as needed.Collaborates with
facility social workers, discharge planners, and case managers to
facilitate safe, early discharge for non-medical
assistance.Consults with physicians, nurses, social workers,
discharge planners and other disciplines to establish a
coordinated, non-medical service delivery plan.Interviews the
client and caregiver and discusses current needs and psychosocial
factors that are relevant to the plan of care.Completes referral
information that includes intake data, essential background
information, mobility aspects, and the proposed elements of the
service delivery plan.Maintains liaison relationship with
hospitals, nursing homes, assisted living facilities, and insurance
personnel providing education on services, coverage issues and
related areas.Participates is establishing the service delivery
plan, client care conferences, and inservices.Attends meetings and
participates on committees as required.Is available as scheduled
for after-hours on-call.Ensures that the on-call phone is
operational and returns calls in a timely manner.Responds promptly
and courteously to client and employee complaints and
inquiries.Directs daily marketing operations including direct
oversight of the establishment and implementation of marketing
initiatives.Employs marketing and promotional initiatives to
achieve budgetary volume projections.Maintains and presents a
professional image through appropriate dress, pleasant and helpful
telephone manner, and courteous interaction with all staff and
visitors; and is responsible for appropriate verbal and behavioral
interaction with all individuals that he/she comes in contact
with.Must be able to perform responsibilities with minimal
supervision, and must possess a high degree of initiative, and the
ability to organize and prioritize responsibilities.Exhibits a high
degree of commitment to customer service and quality of work.19. -
